American Telemedicine Association

2/2021

 Current Statistics:

  • 76% of healthcare is already connected with telemedicine
  • 34 million Americans still lack adequate broadband
  • 40% digital divide for those living in rural areas

  Changes Required:

  • Widespread elimination of geographic and setting location requirements (rural areas)
  • Expanding the types of technology that can be used (remote monitoring)
  • Coverage of service (severe limitations on approved services)

  FCC Rural Health Care Program

  • Supports broadband adoption but there is an insufficient level of subsidy to remote health care providers.
  • Proposal by FCC: Connected Care Pilot: greater investment in broadband access

  Challenges:

  • Unpredictable federal and state legal and regulatory issues
  • Coverage and payment
  • Health professional licensure
  • Credentialing and privileging
  • Online prescribing, including controlled substances
  • Medical malpractice and professional liability insurance
  • Privacy and security
  • Fraud and abuse

  Advocations:

  • Expansion of Medicare coverage with adequate reimbursement that takes into consideration the nursing and other costs incurred at the site where the patient is located
  • The Centers for Medicare and Medicaid Services (CMS) should include telehealth waivers in all its demonstrations and adopt a more flexible approach to adding new telehealth services to Medicare (current cap:  reimbursement for 2 telemedicine visits at $25-$30)
  • CMS excludes rural health clinics and federally qualified health centers
  • Resolution of legal and regulatory challenges that hinder the provision of telehealth services. (different in every state)  Improved access to broadband for rural areas

InjurySnap

 Objectives and Potential Solutions

  • Collaborate with local administrators, government agencies, and healthcare providers in rural geographic areas to determine solutions to expand telemedicine to rural areas. Connect with decision makers to discuss ideas and gain insight
  • Explore government grants and assistance in Telehealth and Telemedicine (specific to rural areas)
  • Partner with charitable organizations or consider organizing one that assist in funding projects to expand Telemedicine
  • Investigate options for bids with the FCC or partnership opportunities to provide broadband/narrowband options for rural area
  • Contact those with knowledge or do online research for up-to-date information on the state and federal legal and regulatory issues, along with potentials to participate in encouraging the CMS to adopt more flexible additions to telehealth in Medicare
  • Develop systematic platforms, as in mobile applications and web portals that can be customed designed to connect patients and providers.
  • Develop features and capabilities that allow for reporting and monitoring of the legal and regulatory challenges, such as privacy and security, fraud, abuse, along with medical malpractice and professional liability.
